The research described in this thesis was carried out at the Netherlands Institute for Health Services Research (NIVEL) in Utrecht and at the Faculty of Social and Behavioural Sciences of the University of Amsterdam. NIVEL participates in the Netherlands School of Primary Care Research (CaRe), acknowledged by the Royal Dutch Academy of Science (KNAW).
