In summary, we showed that smoking or having smoked in the past is an independent risk factor for increased asthma severity within 2 years a er asthma diagnosis. The more pack years smoked, the higher the risk of increased asthma severity. Lung func on, sinus disease and type of airway in amma on at the me of asthma diagnosis are not associated with more severe asthma a er 2 years. Since (ex)-smokers with new-onset asthma are at increased risk of developing severe asthma, these pa ents should be coached intensively for stop-smoking, monitored closely, and receive aggressive preven ve therapy. Since inhaled cor costeroids are not very e ec ve in these pa ents, novel targeted therapies need to be developed for this highly prevalent category of pa ents.
