were optimized according to contemporary guidelines and titrated at the discretion of the treating physician.10,11 Cardiac resynchronization therapy (CRT) and implantable cardioverter defibrillator (ICD) were implanted during follow-up in accordance with current guidelines.12-14 The institutional review board of the Leiden University Medical Center approved this retrospective analysis of clinically acquired data and waived the need for patient written informed consent (C13.029). All data used for this study was acquired for clinical purposes and handled anonymously.
