The survey was conducted from Nov 2014 to Jan 2015.
Randomization of items or questionnaires
No items or questionnaires were randomized.
Adaptive questioning
Adaptive questioning was used twice: the questions on imiquimod use in VAIN and CIN were only shown to those participants that actually applied imiquimod.
Number of Items
The full survey consisted of 37 items. If the participant never applied imiquimod in CIN or VAIN (the expected majority of respondents), only 12 questions were asked.
Number of screens (pages)
The survey consisted of 7 pages, with the amount of questions per page varying from 2 to 13.
Completeness check
The 12 questions that were asked to all participants were all mandatory (this was an automatic option in the survey programme). The questions that were not asked to all participants (but only those that applied imiquimod), were not mandatory as they were not all applicable in all cases.
Review step
Respondents were able to review and change existing answers until the survey was finished or until they closed the survey, through a ‘back button’. Reviewing or changing questions after closing the survey was not possible.
